AHEAD

Introducing a Geriatric Comprehensive Assessment in HEart failure patients in need for a left ventricular Assist Device and heart transplantation (AHEAD study)

Description

This study investigated patients aged 40 years and older who are being screened for eligibility to receive either a heart transplantation or left ventricular assist device. All patients receive a geriatric consultation as part of the screening procedure. The aim of this study is to assess the presence of patient related, disease related and geriatric determinants and their impact on LVAD outcomes such as mortality and major complications. We hypothesise that the presence of patient related, disease related and geriatric determinants obtained during patient selection, such as cognitive impairment, polypharmacy and reduced mobility, will increase the risk of negative outcomes after LVAD implantation and heart transplantation.

Design description
This retro- and prospective cohort study, consists of two WPs, and is a collaboration between the Geriatric, Cardiology and Cardiothoracic Surgery Department in the UMC Utrecht. The first WP consists of a single center, retrospective cohort study. Every patient who received a LVAD (HeartMate II, HeartMate 3 or HeartWare) between 2011 and 2019 and was at least 40 years old at the moment of implantation, will be included. Outcome data will be collected until 31 December 2020, ensuring every patient is followed for at least one year. The second WP is a single center prospective cohort study. Every patient aged 40 years and older who receives geriatric screening prior to LVAD implantation or heart tranplantation will be included. Patient inclusion started at January 2021 and is still ongoing. The aim of this prospective study is to determine the prevalence of patient related, disease related and geriatric determinants in patients with a LVAD/ heart transplantation before and after implantation. And to investigate how these determinants at baseline impact risk of (negative) clinical outcomes at follow up.
